Job summary

ANX Hospice Care in the San Mateo/SF County area seeks a Visiting Registered Nurse (RN) to answer calls, assess patients, and visit per the plan of care. The RN supports patients and families following established hospice protocols.

The role requires a CA RN license and CPR, with at least 1 year of RN experience (hospice preferred). Full-time PM shift; driving required; EMR background helpful for charting and care coordination.

Qualifications

• CA RN license and CPR card required.
• Minimum 1 year RN experience, hospice preferred.
• Full-time PM shift in San Mateo/SF counties; driving required.
• Proficient with EMR and basic computer skills.

Responsibilities

• Review referrals and coordinate hospice eligibility with Director of Patient Care.
• Ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide a physical assessment and gather social/economic data affecting care.
• Coordinate patient care for assigned caseload and ensure comfort and quality of life.
• Apply nursing principles in treatment of patients and provide education to patients and families.
• Encourage patient/family participation in the plan of care to meet goals.
• Coordinate with hospice physicians for in-person or virtual visits.
• Participate in on-call duties and holiday rotations.
• Supervise LVNs and HHAs in following the patient’s plan of care.
• Trained on hospice admissions as a backup.
