Job summary

Kaiser Permanente is seeking a Hospice Visiting Nurse to provide comprehensive skilled nursing services in patients' homes across East King County (Redmond, Kirkland, Bothell, Woodinville, Kenmore). The role covers palliative and hospice care with a rotating weekend schedule, 32 hours per week, and day shift.

Minimum two years RN experience in hospice or home health is preferred; an Associates degree in Nursing and Washington RN license are required.

Qualifications

Minimum two (2) years of RN experience in medical surgical, home health or hospice.
Current KP Coalition employees have this experience waiver.
Associates degree in Nursing from an accredited program.
High School Diploma OR GED required.

Responsibilities

Communicates and collaborates with the interdisciplinary team and patient family; documents data in the electronic medical record; adheres to infection control and safety standards.
Provides skilled nursing interventions, educates patient/caregiver, supervises Home Health Aide and LPN; performs treatments and teaching per plan of care.
Develops plan of care with the team; ensures smooth transition back to primary care providers as needed.
Assesses care needs, collects visit data, and communicates with medical doctors to establish and adjust the plan of care.
