Job summary

Kaiser Permanente in Bothell, WA seeks a Visiting Nurse to deliver comprehensive skilled nursing care in patients' homes as part of an interdisciplinary team in home health, palliative care, and hospice programs. The role requires an ADN and an active WA RN license, with hospice experience preferred.

You will perform skilled procedures, coordinate with families and providers, document in the EMR, supervise LPNs and HHAs, and implement plans of care with clear patient education and safety in mind.

What You'll Do

• Communicate and collaborate with interdisciplinary team members and patient families in planning and implementing care.
• Record accurate and pertinent data in the electronic medical record in a timely manner.
• Provide nursing interventions requiring substantial and specialized nursing skill.
• Follow through the plan of care to provide appropriate instruction to patients/caregivers.
• Direct and supervise the care and services of LPN and HHA in the home setting.
• Perform skilled procedures including injections, dressings, catheter insertion and care, and blood draws.
• Assess patient need for care and communicate with other health care providers appropriately.
• Develop and initiate the plan of care based on assessment.
• Evaluate patient response to care through timely assessment and observation.
• Modify plan of care based on patient and care systems response.

Requirements

Education:

• Associate's Degree in Nursing (ADN) required

Experience:

• Prior hospice or related work experience highly preferred

Licenses & Certifications:

• Current RN License in Washington
