Favorite Healthcare Staffing Inc. is seeking an experienced Registered Nurse (RN) for a full-time Home Health position in Bellevue, WA. The role provides skilled nursing care in patients' homes across King County, with scheduling Monday–Friday and on-call as assigned.

Responsibilities include assessments, plans of care, patient education, documentation, and coordination with other professionals. Must hold WA RN license, travel ability, and reliable vehicle; mileage reimbursed and benefits

Qualifications

• Current Washington State RN license.
• 1+ year of nursing experience preferred.
• Home health experience preferred.
• Valid driver's license and reliable vehicle.
• Ability to travel throughout King County.

Responsibilities

• Provide skilled nursing care to patients in their homes according to physician orders and individualized plans of care.
• Complete initial and ongoing nursing assessments and develop/revise plans of care.
• Monitor patient conditions, communicate changes to physicians and coordinate needed services.
• Educate patients and families regarding care, treatment and health needs.
• Complete clinical documentation, OASIS assessments and required Medicare documentation.
• Coordinate care with PT, OT, SLP, social workers and other healthcare professionals.
• Supervise and provide guidance to LPNs and CNAs as assigned.
• Participate in patient care conferences and quality improvement activities.
