Job summary

Masonicare Home Health & Hospice in Bristol, CT is seeking a dedicated RN Hospice Nurse for day shift coverage. You will deliver high-quality hospice care in patients’ homes or assisted living settings, while ensuring safety and comfort for clients and families.

You will develop and evaluate the plan of care, coordinate with health professionals, supervise aides as needed, and document care using our electronic system. Strong clinical skills and compassionate communication are essential.

Qualifications

• Current RN License required.
• Hospice and/or Home Care experience preferred.
• New grads will not be considered.

Responsibilities

• Provide hospice nursing care in the home or assisted living setting.
• Develop, implement and evaluate the plan of care and revise as needed.
• Coordinate care with other health professionals to ensure continuity of client care.
