Masonicare Home Health & Hospice in Middletown, CT is hiring an RN Hospice Visiting Nurse for day shift, 40 hrs/week, in-home or assisted living settings. The role delivers high-quality hospice care, develops and revises care plans, and coordinates with the care team to ensure continuity of patient care.

Current RN license required; hospice or home care experience preferred. NEW GRADS WILL NOT BE CONSIDERED.

Qualifications

• Current RN license required.
• Hospice and/or home care experience preferred.
• New grads will not be considered.

Responsibilities

• Develop, implement and evaluate the plan of care for hospice patients in home or assisted living settings.
• Coordinate the plan of care with other health professionals and supervise licensed staff as required by regulation.
• Educate and counsel clients and families on home nursing procedures and care needs.
