Job summary

Masonicare Home Health & Hospice is seeking a Registered Nurse Case Manager to deliver high-quality nursing services in patients’ homes in the Middletown, CT area. The role emphasizes care coordination and comprehensive plan-of-care management during the day shift, with some weekend rotation.

Qualified candidates have at least 1 year of clinical experience; new grads are not considered. On-the-job training is provided to support clinical practice and patient outcomes.

Qualifications

• Minimum 1 year clinical experience is required.
• New grads will not be considered.
• On the job training will be provided.

Responsibilities

• Ensure quality and safe delivery of nursing services to patients in the home setting.
• Develop, implement and evaluate the plan of care and revise as needed.
• Coordinate the plan of care with other health professionals to maintain continuity of client care.
• Supervise Licensed Practical Nurses and Home Health Aides and perform supervisory visits as required.
