Job summary

Masonicare Home Health & Hospice seeks an RN Hospice Visiting Nurse in Bristol, CT. The role covers home and assisted living settings, focusing on plan-of-care development, coordination with the care team, and ongoing patient evaluation.

You will document electronically, educate families, supervise LPNs and aides, and ensure alignment with regulatory guidelines while addressing patient needs with compassion and professionalism. Competitive pay and weekly pay schedule available.

Qualifications

• Current RN license required and active.
• Hospice and/or home care experience preferred.
• New grads will not be considered.

Responsibilities

• Develop, implement and evaluate the plan of care for patients at home or in assisted living settings.
• Coordinate the plan of care with other health professionals to ensure continuity of care.
• Supervise and delegate duties to LPNs and Home Health Aides and conduct supervisory visits.
• Provide education and counseling to patients and families on home nursing procedures.
• Maintain timely electronic documentation of patient information and reports.
