Job summary

St. Croix Hospice is seeking a Per Diem Triage Registered Nurse to respond to overnight and weekend calls, assess patient needs, and coordinate hospice services by phone. Responsibilities include documenting in EMR, communicating with physicians, and guiding care plans to support comfort and dignity.

The role requires an active RN license, with possible multistate licensing options. Scheduling includes select weekends and overnight shifts to cover patient needs while maintaining HIPAA compliance

Qualifications

• RN license in state(s) of practice required.
• Compact license acceptable depending on state.
• Ability to pass background and drug tests.

Responsibilities

• Answer calls, assess patient needs, and coordinate hospice services by phone.
• Document care plans and changes in EMR with accuracy.
• Communicate with physicians and families to support patient comfort.
• Dispatch RN/LPN visits when needed and ensure HIPAA compliance.
