Job summary

St. Croix Hospice is seeking a Registered Nurse On-Call to join our 24/7 care model. You will respond to triage calls, perform after-hours visits, and update care plans for eligible patients on an 7 days on / 7 days off overnight schedule.

Ideal candidates have an accredited nursing degree and 1+ years of acute care experience, with an active RN license and CPR certification. You will provide education, medications, and compassionate support to patients and families.

Qualifications

• Graduate of an accredited nursing program (ADN/BSN/MSN).
• 1+ years of recent acute care experience required; hospice or home care preferred.
• Current RN license in state of practice and CPR certification.

Responsibilities

• Respond to triage calls and provide after-hours nursing visits for symptom management, admissions, and care plan updates.
• Develop and adjust individualized care plans based on evolving needs.
• Administer medications and education; maintain timely and accurate clinical documentation.
