Job summary

St. Croix Hospice in Mendota Heights, MN is seeking a Per Diem Triage Registered Nurse to respond to overnight and weekend calls, assess patient needs, and coordinate hospice services by phone.

The RN will document care, communicate with physicians, and support families while maintaining HIPAA privacy and high clinical standards in a home-based setting.

This role requires current RN license, strong nursing experience in palliative care, and the ability to work a flexible weekend schedule.

Qualifications

• Must hold a current RN license in state(s) of practice.
• Experience in hospice or palliative care is preferred.
• Ability to pass background and drug screening.

Responsibilities

• Respond to overnight and weekend calls regarding hospice patients.
• Assess patient needs and coordinate care via phone conference.
• Document care details and communicate with physicians as required.
• Dispatch visits by RN or LPN as appropriate and ensure HIPAA compliance.
• Support families and caregivers to meet emotional and practical needs.
