Job summary

Gentiva is seeking a Hospice RN - Regional Support to deliver compassionate nursing care across multiple hospice locations. You will manage patient care plans, educate families, and collaborate with a cross‑functional team while traveling as needed to fill urgent staffing gaps.

This role includes on-call responsibilities and a Monday–Friday cadence with travel within assigned regions. RN licensure, CPR, and 1+ year hospice/home health experience are required.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience in hospice or related settings.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Deliver high-quality, compassionate nursing care for hospice patients.
• Manage and implement individualized plans of care.
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess and address patients' physical, emotional, spiritual, and environmental needs.
• Accurately document all care provided and ensure compliance with hospice protocols.
• Educate families and caregivers on care instructions, medications, and safety.
• Participate in the on-call rotation and provide after-hours care as needed.
• Collaborate with physicians, referral sources, and the care team.
• Assist with referrals, regulatory documentation, and payer compliance.
• Travel to various Gentiva hospice locations as assigned to fill urgent needs.
