Job summary

Gentiva Hospice seeks an experienced Hospice RN – Regional Support to deliver compassionate nursing care and implement individualized plans of care across multiple locations.

You will collaborate with LPNs, CNAs, HHAs, and physicians, documenting care and educating families. Travel to Gentiva locations is required to fill urgent needs, with a Monday–Friday schedule and occasional on-call responsibilities.

Qualifications

• Current, unencumbered RN license in the state of employment.
• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver high-quality, compassionate nursing care for hospice patients.
• Manage and implement individualized plans of care.
• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.).
• Assess and address patients’ physical, emotional, spiritual, and environmental needs.
• Accurately document all care provided and ensure compliance with hospice protocols.
• Educate families and caregivers on care instructions, medications, and safety.
• Participate in the on-call rotation and provide after-hours care as needed.
• Collaborate with physicians, referral sources, and the care team.
• Assist with referrals, regulatory documentation, and payer compliance.
• Travel to various Gentiva hospice locations as assigned to fill urgent needs.
