Job summary

Enhabit Home Health & Hospice is seeking a sales-focused professional in Lawrenceville, GA to promote home health services and coordinate with patients, families, and providers. The role involves representing the branch in external activities and educating communities about Enhabit offerings.

The ideal candidate will have a clinical education background, be licensed in Georgia, and have at least one year of home care experience. Strong communication and mobility are essential.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws governing hospice operations.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts with patients and families to inform them of Enhabit services.
• Develop patient-centered transition plans integrating clinical guidelines and metrics to promote quality and efficiency.
