Job summary

St. Croix Hospice is seeking a Hospice Transition Nurse in Neenah, WI to serve as a resource RN for patients, families, staff, and referral sources from hospitals and nursing homes. The role focuses on education and expanding referrals through hospital and community partnerships.

Responsibilities include case management, goals of care discussions, care coordination, and mentoring other RNs. A valid RN license in practice states and ability to pass required screenings are essential.

Qualifications

• RN license in state(s) of practice required.
• 1+ years of hospice/home care experience preferred.
• Multistate license may be used without additional licenses in some states.
• Graduate of a professional school of nursing and licensed/certified/registered.
• Ability to pass DHS background, consumer report, and drug test.

Responsibilities

• Acts as case manager for patients in the hospital and manages GIP patients as appropriate.
• Conducts initial goals of care meetings with patients and families.
• Collaborates with business development to educate facilities and other referral sources.
• Documents visits and phone calls with patients, families, and referral sources.
• Coordinates care transitions from hospital to home or hospice/admission.
• Mentors and trains new RNs for high acuity patient needs.
• Attends Interdisciplinary Care Meetings for patients and families.
