Job summary

LilyCare Hospice, serving southern New Hampshire from Bedford, seeks a Hospice Community Liaison to own the territory and build referral networks. You will cultivate relationships with hospitals, SNFs, assisted living facilities, and physician practices across the assigned territory.

Qualifications

• Two or more years of hospice or home health business development.
• Established referral relationships in southern New Hampshire preferred.
• Valid driver's license, reliable vehicle, and current auto insurance.

Responsibilities

• Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory
• Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the LilyCare model of care
• Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals
• Plan and deliver in-services and community education events, managing the associated marketing budget
