Job summary

LilyCare Hospice in Bedford, NH is seeking a Hospice Community Liaison to build and expand our southern New Hampshire referral base across hospitals, SNFs, and physician practices. This field role travels throughout the territory and reports to leadership.

You will educate referral sources about hospice eligibility, the Medicare hospice benefit, and LilyCare's model of care while developing quarterly territory plans with measurable goals. This is a full-time, field position.

Qualifications

• Two or more years of hospice or home health business development preferred.
• Established referral relationships in southern New Hampshire strongly preferred.
• Valid driver's license, reliable vehicle, and current auto insurance.

• Build and maintain referral relationships with hospitals, SNFs, assisted living communities, and physician practices across the assigned territory.

Hospice Community Liaison

LilyCare Hospice is a community-based hospice serving southern New Hampshire out of Bedford. We care for patients where they live — private homes, assisted living, and skilled nursing facilities. We are a growing, nurse-driven organization, not a national chain. Our Hospice Community Liaison owns the territory outright and reports directly to leadership.

Duties and Responsibilities

• Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory
• Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the LilyCare model of care
• Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals
• Plan and deliver in-services and community education events, managing the associated marketing budget

Compensation and Schedule

The Hospice Community Liaison role pays $85,000–$100,000 per year plus performance bonus. The pay range listed represents a general guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience and qualifications. Full-time. Location: 9 Cedarwood Drive, Bedford, New Hampshire 03110, with daily travel throughout the assigned southern New Hampshire territory. This Hospice Community Liaison role is a field position, not a desk position.
