We are a growing, nurse-driven organization, not a national chain. Our Hospice Community Liaison owns the territory outright and reports directly to leadership.

Build and maintain referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices across the assigned territory
Educate referral sources on hospice eligibility, the Medicare hospice benefit, and the LilyCare model of care
Develop and execute a quarterly territory plan with defined target accounts and measurable referral goals
Plan and deliver in-services and community education events, managing the associated marketing budget
The Hospice Community Liaison role pays $85,000–$100,000 per year plus performance bonus. The pay range listed represents a general guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate's relevant experience and qualifications. Full-time. Location: 9 Cedarwood Drive, Bedford, New Hampshire 03110, with daily travel throughout the assigned southern New Hampshire territory. This Hospice Community Liaison role is a field position, not a desk position.
Two or more years of hospice or home health business development preferred
Established referral relationships in southern New Hampshire strongly preferred
Valid driver's license, reliable vehicle, and current auto insurance
