SCAH-Sutter Care at Home - Bay is seeking a Per Diem/Casual RN for weekend and holiday coverage in a hospice setting. You will assess, coordinate, and deliver nursing care with emphasis on comfort, education, and family support in alignment with hospice philosophy and Sutter Health policies.

The role requires RN licensure, BLS, a valid driver's license, and prior hospice experience. Strong communication and teamwork are essential in this collaborative environment.

Qualifications

• Graduate of an accredited school of nursing.
• RN licensed (California) and BLS certified.
• 2 years recent relevant hospice care experience.
• Able to coordinate and educate patients and families.
• Familiar with PHI privacy and healthcare regulations.

Responsibilities

• Assess, coordinate delivery and evaluate nursing care, including direct patient care and education.
• Develop and manage an optimal interdisciplinary plan of care.
• Educate patients and families in line with hospice philosophy and Sutter Health policies.
• Foster collaborative relationships with patients, families, physicians, and other providers.
• Adhere to policies, standards and laws to promote high-quality patient care.
