The nurse will conduct assessments, coordinate care with physicians and hospice staff, document visits, and maintain a compassionate, independent work approach on a weekend schedule.

Qualifications

• Active nursing license (RN or LVN) in good standing.
• Experience in hospice, home health, or related nursing preferred.

Responsibilities

• Perform scheduled weekend hospice nursing visits.
• Assess patients and manage symptoms.
• Educate patients and caregivers on care plans.
• Communicate patient status to the interdisciplinary team.
