Job summary

VNS Health in New York is seeking a physician to oversee medical care for hospice patients, guiding staff and volunteers to ensure quality care. You will consult with attending physicians and provide medical leadership under general direction, working to optimize prognosis evaluation and symptom management.

The role requires NY licensure, PECOS/OPRA, and experience in hospice/palliative care. You will perform home visits and may sign death certificates, coordinating with the Hospice Team Manager

Qualifications

• Licensed and currently registered to practice medicine or osteopathy in New York State required
• PECOS enrollment required
• OPRA enrollment required
• DEA license required
• Board Eligible/Certified in Internal Medicine, Family Practice, Hospice and Palliative Medicine or related specialty
• Valid driver's license may be required, as determined by needs
• CPR Certification preferred

Responsibilities

• Participates in evaluation and recertification of terminal prognosis at indicated intervals
• Collaborates with interdisciplinary team to develop and revise the plan of care
• Completes documentation of all patient care services provided
• Intervenes when there is a possible revocation and reviews with IDT
• Provides medical expertise on prognosis evaluation and pain/symptom management
• Reviews medication regimens for adverse reactions and inappropriate use
• Communicates with attending/referring physicians regarding prognosis and care transfer
• Performs home visits as directed by the Hospice Team Manager
• Provides general medical care when attending physician cannot or chooses not to
• Educates families about medical management and prognosis within HIPAA guidelines
• Ensures care aligns with hospice regulations and VNS Health policies
• Participates in special projects and other duties as assigned
