Job summary

VNS Health is seeking a board-eligible/certified physician in Internal Medicine, Family Practice, Hospice and Palliative Medicine to lead medical care for hospice patients in New York. You will collaborate with the interdisciplinary team, participate in prognosis evaluations, and oversee pain and symptom management.

You will provide home visits as needed, review medication regimens, and support admissions and transfers while adhering to HIPAA and Hospice guidelines.

Qualifications

• Must hold a Medical Degree and be licensed to practice medicine or osteopathy in New York State
• PECOS enrollment required
• OPRA enrollment required
• DEA license required
• Board eligible/certified in relevant specialties with palliative care knowledge
• Bilingual skills may be required

Responsibilities

• Participates in evaluation and recertification of terminal prognosis at indicated intervals
• Collaborates with interdisciplinary team in PPOC development and revision
• Completes documentation of all patient care services
• Intervenes when revocation occurs and reviews revocations with IDT
• Provides medical expertise to admission and care staff on prognosis and pain management
• Reviews patients' medications for adverse reactions, inappropriate usage, and duration
• Collaborates with attending/referring physicians on transfers and prognosis
• Provides home visits as directed by HTM
• Provides for general medical needs including E prescribing when needed and signing death certificates
