VNS Health in New York, NY is seeking a Hospice physician to lead medical care for hospice patients and guide staff and volunteers to ensure quality outcomes.

The role requires Board certification in Hospice and Palliative Medicine or related specialties, NY licensure, PECOS/OPRA/DEA enrollment, and experience with terminal prognosis evaluation, pain management, and coordination of patient transfers. Clinic and home visits are part of the duties.

Formación

• Bilingual skills may be required.
• Medical Degree required by licensure rules.

Responsabilidades

• Participates in evaluation and recertification of terminal prognosis.
• Collaborates with interdisciplinary team (IDT) in PPOC development and revision.
• Completes documentation of all patient care services.
• Intervenes when a possible revocation and reviews revocations with IDT.
• Provides medical expertise on evaluation of prognosis and pain/symptom management.
• Coordinates with attending, referring, and inpatient physicians on prognosis and transfers.
• Performs home visits as directed by the Hospice Team Manager.
• Provides general medical care not met by the attending physician, including E-prescribing when needed and signing death certificates.
