VNS Health, a leading nonprofit home- and community-based health care organization, seeks a physician to oversee medical care for hospice patients and guide the staff and volunteers. You will consult with attending physicians and work under general direction to ensure quality care.

The role involves evaluating terminal prognosis, developing and revising the physician plan of care with the IDT, and providing expertise in pain and symptom management, medication reviews, and patient documentation.

Competenze

• Licensed and currently registered to practice medicine or osteopathy in New York State
• CPR certification preferred
• Bilingual skills may be required depending on needs

Mansioni

• Participates in evaluation and recertification of terminal prognosis
• Collaborates with interdisciplinary team to develop, implement, and revise the physician plan of care
• Completes documentation of all patient care services
• Intervenes when revocation occurs and reviews with IDT
• Provides medical expertise on admissions, prognosis, pain and symptom management, and medication regimens
• Collaborates with attending/referring/inpatient physicians on prognosis and care transfers
• Provides home visits as directed
• Ensures care within HIPAA guidelines and VNS Health policies
• Participates in special projects and other duties as assigned
