Job summary

VITAS Healthcare in Moraine, OH is seeking a Team Manager to lead the Patient Care Team and coordinate interdisciplinary care from admission to discharge. The role focuses on patient advocacy, care continuity, and ensuring high-quality documentation and outcomes within our hospice program.

The position requires RN licensure in the state, a Bachelor’s degree or higher preferred, and at least two years of supervisory experience in healthcare.

Qualifications

• Current and valid RN license to practice in the state where the VITAS program is located.
• Bachelor's degree preferred.
• Two years supervisory experience in a health care organization.

Responsibilities

• Directs all patient care services for the team and ensures integrated care.
• Reviews admissions and discharges to ensure eligibility and appropriateness.
• Collaborates with physicians for pain and symptom management.
• Maintains complete and accurate patient records for recertification decisions.
• Manages team schedule and coordinates 24/7 service delivery.
• Facilitates team meetings and interdisciplinary input into care plans.
