You will follow assessments and care plans while coordinating with nurses, social workers, chaplains, and a care team to ensure safe, compassionate support.

Qualifications

• Strong commitment to improving quality of life while partnering with community organizations in patient care.
• Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and STNA leadership.
• Eager to gain a higher level of opportunities such as certifications and further education.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Able to work independently and on your own on a day-to-day basis.
• Must be willing to drive with reliable transportation, a valid driver’s license, and auto insurance.
• State Tested Nursing Assistant certification in the state(s) of practice.
• CPR certified.

Responsibilities

• The STNA provides resident care under the direction of a Case Manager/DON and follows assessments and care plans.
• Coordinates proper types of care with licensed staff and adheres to nursing policies and safety guidelines.
