Job summary

Patriot Hospice in Ohio is seeking a dedicated Hospice STNA to join our family-owned home health hospice network. This role focuses on compassionate patient care and documentation, with a supportive team environment.

As a Hospice Aide, you will provide personal care, monitor vitals, and help families cope. We offer a flexible schedule, mileage reimbursement, and comprehensive benefits to support staff and their loved ones.

Qualifications

• 75 hours of classroom and supervised practical training completed
• One year experience as a CNA, hospice aide, or home health aide
• At least 18 years old
• Must have valid driver's license with insured vehicle
• Strong verbal and written communication skills

Responsibilities

• Provide personal care according to the Hospice Aide Plan of Care
• Document care provided according to agency policy
• Report any deviations from the Plan of Care
• Take vital signs
• Provide respite for patients' family/caregiver when on-site
• Participate in IDG meetings as appropriate

Business Hours

Monday - Friday, 8:00 AM to 4:30 PM

How we support you professionally:

• This position is paid at a competitive hourly rate, with additional earning potential available through opportunities to pick up visits at our Per Diem rate
• Case Loads of 12-15 Patients
• Mileage reimbursement for any company travel

As a Hospice Aide, you will:

• Perform personal care according to the Hospice Aide Plan of Care
• Document care provided according to agency policy
• Report any deviations from the Plan of Care
• Use DME safely
• Take vital signs
• Provide respite for patients family/caregiver when on-site, as appropriate
• Participate in IDG meetings, as appropriate

Qualifications:

• Received a minimum of 75 hours of classroom and supervised practical training with at least 16 hours of devoted to supervised practical training and with at least 16 hours of classroom instruction completed.
• One year experience as a CNA, hospice aide, or home health aide.
• At least 18 years old.
• Must be a licensed driver with an insured automobile in good working order.
• The ability to communicate well, both verbally and in writing.
