Job summary

Patriot At Home in Middletown, OH is seeking a Home Health Aide (STNA) for PRN visits within a 30-mile radius. The role is paid per diem, reflecting the typically variable schedule for in‑home care.

You will follow the plan of care, assist with personal hygiene, mobility, and safety, and support therapy staff in rehabilitative activities. The team emphasizes independence and compassionate, community‑focused care.

Qualifications

• High school diploma or equivalent required.
• Must hold a valid STNA license.
• Must have completed a minimum of 75 hours of training including an introduction to personal care services.
• Must be free from health problems that may be injurious to patient, self and co-workers.
• Must have current CPR (BLS) license.
• Must have criminal background check.

Responsibilities

• Follows the plan of care to provide safe, competent care to the client.
• Helps maintain the patient to maintain good personal hygiene and assists in maintaining a healthful, safe environment.
• Assists the patient with ambulation as ordered by the physician and approved and supervised by the nurse.
• Assists the therapy personnel as needed with rehabilitative processes.
• Encourages the patient to become as independent as possible according to the nursing care plan.
• Reports any change in the patient's mental or physical condition or in the home situation to the staff nurse.
• Conforms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated.
• All other duties as assigned.

• Must have current CPR (BLS) license
• Must have criminal background check

Job Conditions:

• Valid driver's license required
• Ability to access patients' homes, which may not be wheelchair accessible.
• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care
• Standard aide activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
