Job summary

Patriot Hospice is seeking a Hospice Aide to provide personal care under the direction of a registered nurse in our Ohio service area. The role covers Trumbull, Mahoning, and Columbiana Counties, with a Monday–Friday schedule from 8:00 AM to 4:30 PM.

Qualifications

• Minimum of 75 hours classroom and supervised practical training with at least 16 hours of supervised training and 16 hours of classroom instruction.
• One year experience as a CNA, hospice aide, or home health aide.
• Must be 18 years or older.

Responsibilities

• Perform personal care according to the Hospice Aide Plan of Care.
• Document care provided according to agency policy.
• Report any deviations from the Plan of Care.
• Use DME safely.
• Take vital signs.
• Provide respite for patients' family/caregiver when on-site, as appropriate.
• Participate in IDG meetings, as appropriate.
