Job summary

Patriot At Home in Moraine, OH is seeking a Home Health STNA for PRN per diem visits across a 30-mile radius. You will follow the plan of care to deliver compassionate in-home care, assist with hygiene, ambulation, and safety, and support rehabilitation with the nursing team.

We value reliability and community service; valid STNA license and CPR are required, along with a clean background. This role offers flexible scheduling and the chance to make a real impact in patients' daily lives.

Qualifications

• Preferably a high school diploma or equivalent
• Must have valid STNA License
• Must have completed a minimum of 75 hours of training which includes an introduction to personal care services
• Must be free from health problems that may be injurious to patient, self and co-workers and must present appropriate evidence to substantiate this
• Must have current CPR (BLS) license
• Must have criminal background check

Responsibilities

• Follows the plan of care to provide safe, competent care to the client
• Helps maintain the patient to maintain good personal hygiene and assists in maintaining a healthful, safe environment
• Assists the patient with ambulation as ordered by the physician and approved and supervised by the nurse
• Assists the therapy personnel as needed with rehabilitative processes
• Encourages the patient to become as independent as possible according to the nursing care plan
• Reports any change in the patient's mental or physical condition or in the home situation to the staff nurse
• Conforms on a weekly basis, the scheduling of visits so that other necessary visits by staff members can be coordinated
• All other duties as assigned

• This position is paid at a competitive per diem visit rate

The responsibilities of our Home Health Aides include:

Job Conditions:

• Valid driver's license required


• Ability to access patients' homes, which may not be wheelchair accessible.


• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care


• Standard aide activities that include walking, sitting, stooping, standing, and varying lifting/moving/turning of patients
