Advocate Health Hospice Union in Monroe, NC is seeking a qualified spiritual care provider with a strong theological background to join our patient and family services team. The role involves assessing spiritual needs, offering counseling and sacraments, and collaborating with the interdisciplinary hospice team to implement a personalized plan of care.

Applicants should have a Master's degree in Theology/Divinity/Religious Studies, ACPE CPE units (2+), and be comfortable traveling to patient

Qualificações

• Master's degree in Theology, Divinity, or Religious Studies required.
• ACPE; at least 2 CPE units required, 5 units preferred.

Responsabilidades

• Provides initial spiritual assessment and ongoing care to patients and families.
• Delivers pastoral counseling, sacraments, and support aligned with patient faith.
• Develops spiritual plan of care with hospice team and executes per protocols.
• Facilitates end-of-life conversations including goals of care and legacy work.
• Collaborates with hospice team for referrals in crises and participates in QA/CQI processes.
