Job summary

Advocate Health in Monroe, NC seeks a full-time Spiritual Care Provider to support patients and families with pastoral care, counseling and spiritual resources. You will assess needs, develop care plans, and coordinate with the hospice interdisciplinary team, honoring each patient’s faith and preferences.

Weekly schedule is Monday–Friday 8:30am–5:00pm with on-call weekend rotation; travel to patient homes and facilities; IRS mileage reimbursement; 40 hours per week; requires Master’s in

Qualifications

• Master's degree in theology/divinity/religious studies required or equivalent.
• At least 2 CPE units; 5 units preferred; ACPE/NACC endorsement encouraged.
• In good standing with church or religious institute; Notary within 6 months.

Responsibilities

• Provide spiritual care and emotional support to patients and families.
• Develop a spiritual plan of care with interventions and outcomes.
• Collaborate with hospice team and faith communities; support end-of-life conversations.
• Participate in QA/CQI processes and travel to patient homes and facilities.
