Kindful Health LLC seeks a Chaplain to provide spiritual care in patients' homes in Bastrop, TX. The primary function is to assess spiritual needs and offer counseling to patients and families.

The chaplain will collaborate with the Interdisciplinary Group/Team (IDG/IDT), attend meetings, and contribute to care planning. A Master of Divinity is preferred and one year of hospice chaplain experience is desirable.

Qualifications

• Master of Divinity preferred.
• One year of chaplain experience in a healthcare setting preferred.
• Knowledge of community spiritual resources and strong interpersonal skills.

Responsabilités

• Assess Hospice patient and support system to identify spiritual needs.
• Provide support for the spiritual needs of patient and family.
• Attend IDG/IDT meetings and participate in care planning.
• Contribute to Quality Assessment and Performance Improvement processes.
