Job summary

Kindful Health in Bastrop, TX seeks a Spiritual Counselor/Chaplain to provide in-home spiritual care for terminally ill patients and their families. The role includes assessing spiritual needs, coordinating with the IDG/IDT, and guiding care plans in alignment with the Hospice philosophy.

The position requires Master of Divinity (preferred), healthcare chaplain experience (preferred), and reliable transportation.

Qualifications

Education: Master of Divinity preferred.
Licensure: Not Applicable.
Experience: One year of experience as a Chaplain in a healthcare institution preferred.
Skills: Working knowledge of community spiritual and other resources.
Transportation: Reliable transportation; license/insurance if personal vehicle used.

Responsibilities

Assess the Hospice patient and support system to identify spiritual needs and document in the Plan of Care.
Provide support for the spiritual needs of patient and family.
Attend IDG/IDT meetings and participate in patient care planning.
Participate in Quality Assessment Performance Improvement process as required.
Demonstrate effective interpersonal relations and communication to coordinate care.
Meet mandatory continuing education requirements and stay informed on eligibility standards.
Promote Hospice philosophy and administrative policies to ensure quality of care.
Collaborate with IDG/IDT to coordinate care and services.
