Job summary

• Experience: One year of experience as a Chaplain in a healthcare institution preferred.

Responsibilities

• Assess the Hospice patient and support system to identify spiritual needs as evidenced by the Plan of Care (POC).
• Attend IDG/IDT meetings and participate in patient care planning process.
• Engage in Performance Improvement as required.
• Use effective interpersonal relations and communication skills to support the team.
• Maintain continuing education requirements for Hospice.
• Promote Hospice philosophy and policies to ensure quality care.
• Coordinate with IDG/IDT for continuity of care.
• Accept clinical assignments aligned with education and competence.
