Job summary

Central Vermont Home Health & Hospice seeks a Spiritual Care Coordinator to provide spiritual care to clients and families and to coordinate referrals and home or facility visits. You will work closely with an interdisciplinary team to ensure compassionate, patient-centered care throughout the hospice journey.

Qualifications

• Graduate of an accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry required.
• Minimum one year experience as an ordained pastor/rabbi or one year of hospice experience

Responsibilities

• Coordinate referrals and home or facility visits for hospice patients and families.
• Collaborate with the hospice interdisciplinary care team.
• Provide grief support and bereavement follow-up services.
• Counsel staff and families through emotionally complex situations.
• Engage with clergy and community groups to address spiritual needs.
• Facilitate memorial services when appropriate.
