Job summary

Vitality Hospice & Palliative Care is seeking a dedicated Hospice Chaplain to provide compassionate spiritual care to patients, families, and caregivers. In collaboration with the Interdisciplinary Team, you will develop and document individualized spiritual care plans and participate in care conferences to ensure holistic, culturally sensitive support.

This full-time role requires interfaith sensitivity, strong communication, and the ability to work in a dynamic, on-the-road environment.

Qualifications

• Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counseling.
• Experience working with death and dying.
• Ability to work effectively within an interdisciplinary team.
• Comfort working in an interfaith, multicultural environment.

Responsibilities

• Assess spiritual needs and provide care to patients and families.
• Develop, implement, and document individualized spiritual care plans.
• Collaborate with the Interdisciplinary Team to coordinate holistic care.
• Participate in IDT meetings and care plan reviews.
• Provide consultation, education, and support to staff regarding spiritual care.
• Coordinate with clergy and community spiritual resources as requested.
• Conduct pre-bereavement and bereavement assessments and visits.
• Facilitate funeral and memorial services as requested.
• Supervise spiritual care volunteers as assigned.
• Manage caseload, scheduling, and travel efficiently.
