Job summary

Vitality Hospice & Palliative Care seeks a dedicated Hospice Chaplain to provide compassionate spiritual care to patients and families. This on-the-road role reports to the Clinical Director or designee and operates within an exempt schedule, supporting an interdisciplinary team.

The chaplain maintains spiritual care plans, coordinates with clergy, and participates in care planning while adhering to hospice philosophy and regulatory standards.

Qualifications

• Graduate of an accredited seminary or school of theology (B.D., M.Div., or equivalent), or degree in counseling.
• Experience working with death and dying.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Assess spiritual needs and provide supportive spiritual care to patients and families.
• Develop, implement, and document individualized spiritual care plans.
• Collaborate with the Interdisciplinary Team to coordinate holistic care.
