Job summary

MJHS in Newark, NJ is seeking a licensed MSW to provide psychosocial support and social work services to hospice patients and their families in inpatient units, nursing homes, or home care settings. The role includes developing care plans, coordinating discharge planning, and documenting assessments and interventions.

You will participate in pre- and post-bereavement services, collaborate with an interdisciplinary team, and advocate for patient and family needs.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS driver's license preferred.
• Ability to work within an interdisciplinary team environment.
• Possess excellent organizational, communication and listening skills.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and Social Work services to Hospice patients and their family members who are at an inpatient unit, nursing home or home care setting
• Provide pre and post bereavement services
• Serve as a social work member of hospice IDG and as liaison between hospice, inpatient and nursing home staff
• Develop the social work plan of care, set goals and make recommendations based on the patient and family needs
• Maintain accurate and current documentation reflecting palliative social work assessments andinterventions
• Take part in discharge planning when appropriate for patients admitted into inpatient facility
