Job summary

MJHS is a large not-for-profit health system in the Greater New York area, offering hospice and palliative care services. This social work role provides psychosocial support to hospice patients and families across inpatient units, nursing homes, and home care settings.

You will develop social work plans of care, conduct pre- and post-bereavement services, and coordinate discharge planning while maintaining thorough documentation.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS driver's license preferred.
• Ability to work within an interdisciplinary team environment.
• Excellent organizational, communication and listening skills.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and Social Work services to Hospice patients and their families in inpatient, nursing home or home care settings.
• Provide pre and post bereavement services.
• Serve as a social work member of hospice IDG and liaison between hospice, inpatient and nursing home staff.
• Develop the social work plan of care, set goals and make recommendations based on patient and family needs.
• Maintain accurate and current documentation reflecting palliative social work assessments and interventions.
• Participate in discharge planning when appropriate for patients admitted into inpatient facility.
