Job summary

MJHS is a large not-for-profit health system serving the Greater New York area, offering hospice and palliative care across inpatient units and home settings. This role provides psychosocial support to hospice patients and families, with duties in bereavement services and care planning.

The position emphasizes collaboration with the hospice IDG and other staff, thorough documentation, and active participation in discharge planning as needed.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• Valid NYS Driver's license preferred.
• Ability to work within an interdisciplinary team environment.
• Possess excellent organizational, communication and listening skills.
• Ability to input clinical data proficiently.

Responsibilities

• Provide psychosocial support and Social Work services to Hospice patients and their families at inpatient, nursing home or home care settings.
• Provide pre and post bereavement services.
• Serve as social work member of hospice IDG and liaison between hospice, inpatient and nursing home staff.
• Develop the social work plan of care, set goals and make recommendations based on patient and family needs.
• Maintain accurate and current documentation reflecting palliative social work assessments and interventions.
• Engage in discharge planning when appropriate for patients admitted to inpatient facilities.
