Job summary

MJHS is seeking a social worker to provide psychosocial support and bereavement services to hospice patients and families across inpatient, nursing home, or home care settings. You will be part of an interdisciplinary team, develop plans of care, document interventions, and participate in discharge planning as needed.

Minimum one year in healthcare social work and accredited MSW degree required; NYS MSW licensure and NY driver license preferred.

Qualifications

Graduate from an accredited School of Social Work.
Minimum of one year experience in Social Work in a healthcare setting.
Knowledge of Word and Excel preferred.
New York State MSW licensure preferred.
Valid NYS Driver's license preferred.

Responsibilities

Provide psychosocial support and Social Work services to hospice patients and families in inpatient, nursing home, or home care settings.
Provide pre and post bereavement services.
Serve as a social work IDG member and liaison between hospice, inpatient, and nursing home staff.
Develop the social work plan of care, set goals, and make recommendations based on patient and family needs.
Maintain accurate documentation reflecting palliative social work assessments and interventions.
Participate in discharge planning when appropriate for patients admitted to inpatient facilities.
