Job summary

MJHS is a large not-for-profit health system serving the Greater New York area, offering hospice and palliative care, home care, and related services. This role provides psychosocial support to hospice patients and families, conducts pre- and post-bereavement services, and coordinates with inpatient, home care, and nursing home teams to develop care plans and ensure smooth discharge planning.

The candidate should have an MSW from an accredited school, at least one year in healthcare social work,

Qualifications

• MSW degree from an accredited school.
• Minimum 1 year of social work experience in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.
• New York State driver’s license preferred.

Responsibilities

• Provide psychosocial support and social work services to hospice patients and families at inpatient unit, nursing home or home care setting.
• Provide pre and post bereavement services.
• Serve as social work member of hospice IDG and liaison between hospice, inpatient and nursing home staff.
• Develop the social work plan of care, set goals and make recommendations based on patient and family needs.
• Maintain accurate and current documentation reflecting palliative social work assessments and interventions.
• Take part in discharge planning when appropriate for patients admitted into inpatient facility.
