Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, collaborating with interdisciplinary teams. This role requires hospice experience (1+ years) and licensure as LSW or LCSW in Illinois. A sign-on bonus is offered.

Qualifications

Hospice experience required (1+ years).
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensure per IDFPR with SOS and auto insurance proof as applicable.
Experience working with hospice patients and families in a team-based setting.

Responsibilities

Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis, illness, and life situation.
Develop bereavement care plans addressing family/caregiver needs.
Educate patients and families, overcoming barriers and matching learning styles.
Document psychosocial assessments, financial assessments, and MSW interventions in the EMR.
Collaborate with IDT to develop and implement social work plans of care and adjust as needed.
Identify and respond to end-of-life indicators and support patient/family decisions.
