Emblem Hospice in Scottsdale/Fountain Hills is hiring a full-time Hospice Social Worker to serve patients across Scottsdale and Fountain Hills. You’ll assess psychosocial needs, provide counseling, and collaborate with an interdisciplinary team to ensure dignified, holistic care.

The role features a Monday–Friday schedule and community-based visits. Requirements include an MSW from a CSWE-accredited program, an active AZ social work license, 1 year in healthcare (hospice preferred), CPR

Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Active State of Arizona social work license.
• Minimum 1 year in healthcare; hospice experience preferred.
• Current CPR certification and valid driver's license.

Responsabilités

• Comprehensive psychosocial assessments of patients and families.
• Develop and implement care planning based on evaluations.
• Provide counseling and support to patients and caregivers.
• Maintain accurate, timely clinical records for referred patients.
• Provide referrals and liaise with community agencies.
