Emblem Home Health & Hospice in Phoenix seeks a Hospice Liaison to build relationships with physicians, hospitals, and referral sources, guiding families to compassionate hospice services.

This role focuses on education, outreach, and coordination with care teams to ensure dignity and quality care throughout the patient journey.

Qualifications

• Bachelor's degree in Marketing, Business Admin, or related field.
• Minimum 3 years healthcare marketing, hospice preferred.
• Strong communication and relationship-building skills.
• Ability to work independently with a patient-first approach.
• Reliable transportation and AZ driver's license.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, SNFs, assisted living, and other referral sources.
• Educate healthcare professionals and community groups about hospice services and benefits.
• Serve as a knowledgeable, compassionate resource for patients and families during referral and admission.
• Document outreach activities and maintain accurate records of interactions.
• Ensure all messaging reflects dignity, comfort, and quality care.
• Collaborate with care coordination teams to support smooth patient transitions.
• Monitor community needs and identify opportunities to improve access to hospice care.

As the Hospice Liaison, you’ll serve as a trusted resource in the community, building relationships with healthcare providers and guiding families toward compassionate hospice services. This role is more than marketing, it’s about ensuring dignity, comfort, and support during life’s most challenging moments.

• Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources
• Educate healthcare professionals and community groups about hospice services and benefits
• Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process
• Document outreach activities and maintain accurate records of interactions
• Ensure all messaging reflects our commitment to dignity, comfort, and quality care
• Collaborate with care coordination teams to support smooth transitions for patients
• Monitor community needs and identifies opportunities to improve access to hospice care

Requirements

• Bachelor’s degree in Marketing, Business Administration, or related field preferred
• Minimum 3 years of healthcare marketing experience, hospice experience strongly preferred
• Strong communication, relationship-building, and public relations skills
• Ability to work independently while maintaining a compassionate, patient-first approach
• Demonstrated organizational skills and ability to meet outreach goals
• Reliable transportation, valid AZ driver's license, and proof of insurance
