Emblem Hospice in Chandler, AZ is hiring a compassionate Hospice RN Case Manager to coordinate in-home care across Mesa, Chandler, and Gilbert. This full-time role follows a Monday–Friday schedule and emphasizes patient comfort, family education, and symptom management in a home setting.

You will work with a collaborative interdisciplinary team, conduct comprehensive assessments, develop care plans, and ensure timely documentation and communication with physicians.

Qualifications

• Active Registered Nurse license in Arizona.
• Minimum 2 years nursing experience; hospice or home health preferred.
• Homecare Homebase (HCHB) experience preferred but not required.
• Strong clinical skills and patient-centered care mindset.

Responsibilities

• Perform comprehensive patient and family assessments.
• Develop and implement individualized care plans; revise as needed.
• Provide skilled nursing care, administer meds, and manage symptoms.
• Educate patients and families on care techniques and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality initiatives.
