Job summary

Emblem Hospice in Phoenix, AZ is seeking a dedicated Hospice RN to serve as a trusted Case Manager, delivering compassionate care in patients' homes and communities.

You will guide families through care decisions, provide comfort, dignity, and support, and collaborate with an interdisciplinary team to improve quality of life for patients and families they serve.

Qualifications

• Currently licensed as a Registered Nurse (RN) in Arizona.
• Minimum of 2 years nursing experience; hospice or home health preferred.
• Valid driver’s license with reliable, insured vehicle.

Responsibilities

• Perform comprehensive assessments of patients and families for hospice needs.
• Develop and implement individualized care plans; revise as needed.
• Provide skilled nursing care, administer meds and manage symptoms.
• Educate patients and families on care techniques and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement.
• Provide emotional support to patients and families.
• Supervise hospice aides and delegate tasks appropriately.

_*This is a full-time position offering a Monday-Friday schedule!

*This position requires travel and will provide service throughout the South Scottsdale areas!_

• Complete comprehensive assessments of patients and families to determine hospice needs

• Maintain accurate clinical documentation and communicate changes to physicians

• Provide emotional support and counseling to patients and families
• Supervise hospice aides and delegate responsibilities appropriately

_ Requirements _

• Currently licensed as a Registered Nurse (RN) in the State of Arizona
• Minimum of 2 years of nursing experience, hospice or home health experience preferred
• Homecare Homebase (HCHB) experience preferred, not required
• Valid driver’s license and reliable, insured vehicle
• Strong clinical skills and a passion for patient-centered care
