Job summary

Blue Summit Hospice in Roswell, GA seeks a PRN Social Worker to deliver psychosocial, emotional and spiritual support to patients and families in the Atlanta area. The role includes assessments, counseling, bereavement services and coordination with the interdisciplinary group.

The position requires MSW (CSWE-accredited) with healthcare experience; BSW with MSW supervision may qualify under state rules. On-call and field visits are expected as part of the hospice program.

Qualifications

• MSW degree from CSWE-accredited program with one year of health-care experience.
• BSW from CSWE-accredited program with one year of health-care experience and MSW supervision.
• Related degree in psychology or sociology with one year of health-care experience and MSW supervision (per state rules).

Responsibilities

• Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in development and implementation of the IDG care plan.
• Conducts ongoing reassessments of patient/family needs and counseling as required.
• Provides short term crisis intervention and individual or family counseling when indicated.
• Provides services under the direction of a physician who approves the plan of care.
• Participates as a member of the Bereavement Team as assigned.
• Documents comprehensive psychosocial, emotional and spiritual assessments in the patient record.
• Attends staff meetings and IDG conferences as assigned.
• Adheres to Hospice policies and participates in ongoing professional development.
