Job summary

Hospice seeks a PRN Social Worker to deliver psychosocial and bereavement services to patients and families in the Snellville/Athens area. The role includes initial assessments, ongoing counseling, and coordination with the inter-disciplinary group (IDG) to develop and update plans of care.

Minimum qualifications include an MSW (CSWE) with required healthcare experience or a BSW with MSW supervision; state requirements apply. Strong communication and crisis response skills are essential.

Qualifications

• MSW from CSWE-accredited program with 1 year health-care experience or supervision.
• BSW from CSWE-accredited program with supervision by MSW and 1 year health-care experience.
• Alternative: BA/BS in psychology or sociology with MSW supervision and state compliance.

Responsibilities

• Perform psychosocial, emotional, spiritual and bereavement assessments.
• Provide ongoing counseling and crisis intervention when indicated.
• Develop and maintain IDG care plans with the physician.
• Refer to community resources and coordinate interagency referrals.
• Document assessments and interventions in the patient record per policy.
• Participate in IDG, bereavement team, and staff meetings.
